You must have the following
document to register your child for school.:

Birth Certificate

Social Security Card
-
Health Record (Grades
1-12 must provide proof of physical within the last year;
Kindergarten Students must have proof of physical within the last 6
months.

Immunization Record (shot
record-must say at the top-"Tennessee Immunization Record")

Proof of Residency -
(example: Utility Bill, Rent Receipt, U.S. Mail, etc.)

when registering your child for school.
Please feel free
to print these forms, fill them out in advance and bring them with you
when you come to enroll your child in the Anderson County School System.
